New Patient Checklist

  • Call the clinic to complete a pre-registration and schedule an assessment with intake staff and a medical visit.

  • Complete an assessment with intake staff.

  • Complete a physical, leave a urine sample, and start your medication.

    Transferring from another facility? In order to protect our patient’s privacy and confidentiality under 42 CFR Part 2, please fill out the following Release of Information (ROI) form to enable us to acquire your transfer paperwork.

Things to Remember

  1. Present at the clinic with picture ID and insurance information.*
  2. Show up 5-10 minutes early for your intake appointments.
  3. Follow the recommendations of your medical provider carefully.
  4. Attend a patient orientation within the first four weeks of beginning treatment.
  5. Meet regularly with your counselor.
  6. Start recovery!

*If you have no insurance, we have a navigator onsite who will screen and assist with a Medicaid application when eligible.

Patient Eligibility

Patients must meet certain requirements for admission. We do not accept anyone under the age of 18, patients seeking pain management treatment, or those who do not use opioids.

No matter your level of healthcare coverage – private insurance, Medicare/Medicaid, VA, or even no insurance – we make it possible to start your recovery at WCDC. Financial navigators are available on-site to guide you through paying for treatment. We can even help you qualify for our services with no out-of-pocket costs.

The structure of We Care Daily Clinics is based on highly successful tribal behavioral healthcare programs, but both native citizens and non-native individuals are welcome to enter our MAT program. The Muckleshoot Tribe is committed to helping our neighbors and building a stronger community.
