Our referral process is simple and confidential—designed to connect patients with the right level of support across medical, behavioral, and recovery services.

How to Refer a Patient

  1. Patient Meets Criteria for Treatment
    Patient has a need for opioid use disorder services and has Medicaid or Medicare eligibility.

  2. Submit ROI Electronically or by Fax
    Submit Release of Information online, or download PDF, fill out, and fax to 253-999-5740 (Auburn) or (206) 577-8933 (North Seattle).

  3. Get Scheduled
    Our intake team will follow up with the referred patient within 1-2 business days to schedule an appointment
